Proposed changes to version 2.1: - Based on discussion and vote at the DEM meeting 3/13/07, agreed to eliminate the "NIH Stroke scale 6-19" from the relative criteria. Reason: as a stroke center, we often have additional information that allows us to make an informed decision to give tPA in situations and with stroke scales that are usually not considered appropriate for tPA. - added "Green is the Medical Records original copy. Please retain." - Moved "NIH Stroke Scale 6-19" from Absolute to Relative criteria - Decided that, on the initial BAPET orders, only need one IV; but if we start thrombolytics, then we need three IVs. Changes prior to version 2.0 (7/11/06): - Combined the tPA orders and non-tPA CVA orders into a single order set. - made a flowchart-type process using radio buttons and checkboxes to guide users through making and documenting the decision to give systemic thrombolytics.
