þ I:E Ratio:
 - In most cases, begin with an I:E ratio of 1:2.  Many suggest
   decreasing this ratio to 1:4 or 1:5 in patients with
   obstructive airway disease to avoid air-trapping and auto- or
   intrinsic PEEP with resultant barotrauma
