þ Testing:
  - WBC ESR not useful:
    WBC 10K in 50%ESR > 50 in 70-90%
    cultures not even that useful
    WBC in joint fluid generally > 50,000 (some say 100K with >75% polys)
     (5000-50,000 in gout etc.)
