Intubating ODs ============== þ Salicylate/ASA OD - has respiratory alkalosis; interrupting by sedating or paralyzing, or inadequate vent settings. - need to continue the hypocarbia; check gas before and after, adjust vent up as needed. IMV 12, TV 10 mL/kg totally inadequate - RSI almost always drops pH - actually, same with any severe acidosis þ Organophosphates - anticholinesterase; and adding sux, may turn into long-acting paralytic; case reports of patient with some chronic malathion exposure, no visible toxicity, sux lasted 3 hours. Another case was 7 hours. Not all that bad in the ED but a problem for recovering the patient after the OR.
