OB-Monitoring ============= þ Internal Fetal Monitoring - much better than external - membranes must have ruptured or delivery imminent þ External Fetal Monitoring - requires experienced OB nurse - easier when patient still and supine, but this is not good if patient hypovolemic þ When to monitor fetus? - not before 20 weeks; not viable - if over 20 weeks and any significant traumatic event - monitor for at least 4 hours; extend to 24 hours if maternal injury is serious, fetal heart tracing is worrisome, uterine contractions more frequently than 15 minutes, rupture of membranes, or bleeding. - follow up on fetal monitoring after trauma: ultrasound every 3 weeks to check on fetal growth.
