Methadone
                                =========
þ Street Addicts
 - methadone 15-30 mg daily, divided BID/QID, or
   10 mg PO or 5 mg IV Q6H
 - may need to increase to 40-80 mg daily
 - use OBJECTIVE signs to increase dose
 - don't let patient know dosage, mask in juice
 - can give 1/2-2/3 of oral dose via IV if needed
 - taper 10-15% daily
