Hantavirus ========== þ Epidemiology - Associated with dried rodent urine -- very sturdy virus, can last for many months in dried form. - Found probably everywhere, but at low levels. - Most rodent carriers are healthy - not seen in prarie dogs or pet rodents, at least not yet. - Dead-end virus, cannot be transmitted to healthcare workers. Despite massive exposure of healthcare workers to sputum, etc., no transmission. However, recent outbreak in Bolivia may have þ NBC: - North Korea working on Hantavirus/Influenza and Hantavirus/Influenze combination, using old Soviet scientists. þ Clinical Features - Starts with SOB; in about 6 hour, develops fever, chills, myalgias, sometimes nausea, vomiting, diarrhea. - Rapidly develops respiratory failure with ARDS-like features. þ Treatment - Supportive care only. No vaccine, unlikely to ever be one per CDC. - Do well if mechanically ventilated for a day or two. - Broad spectrum antibiotics until can confirm Hantavirus. - May need pressors for support. - check http://www.cdc.gov/ncid/ for detailed clinical data.
