Fomepizole (e.g., Antizol) ========================== þ Used for - ethylene glycol - methanol - diethylene glucol, triethylene glycol, butoxyethanol -- may be effective þ vs. Ethanol as treatment: - Ethanol: need to monitor to keep level 100 mg/dl to keep alcohol dehydrogenase busy - Ethanol: absorption and metabolism of ethanol erratic - Ethanol: CNS depressant - Ethanol is cheap, one package of fomepizole at MH; cost is $4,600 per package. - Cost comparison: about the same cost if figure out ICU and monitoring/test costs. þ Fomepizole available in veterinary formula - may have lower cost þ Dosing: - dose is 15 mg/kg loading, then 10 mg/kg Q12h for 4 doses, then 15 mg/kg Q12H thereafter until ethylene glycol levels are less than 20 mg/dL. Given BID, or Q4H while on dialysis. Each dose is diluted and given over 30 minutes. Available in 1g/ml, 4 1.5 ml vials per package.
