þ Empyema Etiology
 - can be caused by just about anything infectious including fungi
 - in 1930s-40s, was mostly pneumococcus and strep
 - in 1950s-60s, more Staph and gram negative rods
 - now known to be lots of aneaerobes in the empyemas as well [Chen KY, Hsueh
