2nd Generation: Add H. Flu coverage or Bacteroides coverage, retain Staph and
                  Strep coverage
                Oral: Ceclor
                Parenteral vs. H. Flu but not Bacteroides:  Cefuroxime (Zinacef,
                  Kefurox; 1.5/3.0 g doses); Cefamonadol (Mandol); Ceforanide (Precef);
                  Cefonocid (Monocid).
                Parenteral vs. Bacteroides:  Cefoxitin (Mefoxin); Cefmetazole
                  (Zefazone); Cefotetan (Cefotan) (more Gram + and less
                  anaerboic coverage toward end of list)

3rd Generation: some retain gram + activity (Ceftriaxone: Rocephin; IM/IV
                1-2g daily)
                usual dosage per Hank Freedy, Mercy Pharmacy: 1 g Q24H unless:
                more than 100kg, or meningitis, or pneumonia/effusion, then
                2g/day (but in any case, 2g good loading dose in ED)
                some cover Pseudomonas:  Ceftazidime (Fortaz, Tazicef, Tazidime)
                (avoid Cefobid [Cefoperazone]: poor CNS penetration and
                Moxalactam [Mocam]: interferes with PT and platelets)
