Parenteral vs. Bacteroides: Cefoxitin (Mefoxin); Cefmetazole (Zefazone); Cefotetan (Cefotan) (more Gram + and less anaerboic coverage toward end of list) 3rd Generation: some retain gram + activity (Ceftriaxone: Rocephin; IM/IV 1-2g daily) usual dosage per Hank Freedy, Mercy Pharmacy: 1 g Q24H unless: more than 100kg, or meningitis, or pneumonia/effusion, then 2g/day (but in any case, 2g good loading dose in ED) some cover Pseudomonas: Ceftazidime (Fortaz, Tazicef, Tazidime) (avoid Cefobid [Cefoperazone]: poor CNS penetration and Moxalactam [Mocam]: interferes with PT and platelets)
