  Parafon Forte (Chlorzoxazone):
 - can cause fatal hepatotoxicity
 - per Medical Letter 5/10/96: "Chlozoxazone can cause unpredictable,
   fatal hepatic toxicity, and its efffectiveness as a muscle relaxant is
   questionable.  There is no reason to use it."
