Musculoskeletal Chest Pain ========================== þ Tietze's Syndrome (costochondritis) - characterized by pain and swelling of a single costochondral junction. þ Costernal Syndrome - cause unknown - usually affects multiple costochondral junctions - characterized on physical exam by tenderness without swelling. ref: Tintinalli 3rd ed. p 131 þ Slipping Rib Syndrome - The inferior margins of the 8th to 10th ribs become tender as the inferior rib slips upward and overides the superior rib, producing pain and an audible click. Ref: Tintinalli 3rd ed. p 131 þ Precordial Catch Syndrome - The patient suffers episodes of sharp, short, stabbing precordial pain anteriorly. Rest and shallow respiration relieve the pain. Tintinalli 3rd ed. p 131 þ Thoracic Outlet Syndrome - Classic History: A patient complains of chest, neck and shoulder pain, occasionally with paresthesias of the upper extremities. On physical exam, the supraclavicular fossa is tender. Elevation and exercise of both arms causes paresthesias and weakness in the hands. Hyperabduction and external rotation of the upper extremities brings on the symptoms and weakens the radial pulse. Ref: Tintinalli 3rd ed. p 131
