Brugada Syndrome

þ Epidemiology
 - 1 of every 25 arrests from Brugada syndrome
 - mortality 10% per year
 - ? Sodium channel abnormality
 - Sodium channel blockers make worse
 - Heat makes worse
 - Found in all age groups, all races
 - Usually found at about age 30, sometimes in older people
 - If patient has chest pain with Brugada EKG changes, consider MI instead
 
þ Presentation
 - may present with syncope 
 - or with polymorphic v tach
 
þ EKG changes
 - Look at V1 and V2
 - Incomplete RBBB with ST elevation
 - Normal RBBB has ST _depression_
 - coved ST elevation more specific and sensitive than saddle-type
 
þ Treatment
 - Send to EP lab, check for inducible VT, place ICD
 - medications don't work
