Listed below are several that are good for background info and a few more
that contribute to our discussion.  Two notable inclusions are:

the one by Johnson "...a comparison of two techniques" where, in a too
small study, (44) validated equal success rates with paramedics performing
surg. crics /c "pre-packed kits, ala Nu-Trake.  Interestly they noted that
the surg. procedure was notably faster (46+,- 17sec vs 103=,- 62 sec).

and the Brit. Jour of Anes. "Extra inspiratory work of breathing imposed by
cricothyrotomy devices"  using a lung model for spontaneous ventilation,
they felt that a 12 or 14ga catheter produced an excessive workload, but a
4.0 ET tube was fine and would support ventilation adequately.  This one
rates reading.
