Paralytics ========== þ Sedative/Paralytic Comparison Table þ Succinyl Choline (Anectine) - usual dose 100 mg - OK to use in burns or major trauma up to about 7 days after trauma - OK for head injury or penetrating eye injury þ vecuronium (Norcuron) - Dose: 7 mg 0.1-0.2 mg/kg (0.2 for infants) - Ready to intubate in 4 minutes (0.01-0.02 for defasciculating dose) þ pancuronium (Pavulon) - Dose: 0.1-0.2 mg/kg þ rocuronium (Zemuron) - 70 mg + 0.6-1.2 mg/kg + 2 minute onset - preferred by pediatricians, as will not cause prolonged paralysis as will depolarizing blickers such as succinyl choline in cases of undiscovered muscular dystrophy. þ Raplon (rapacuronium) - 1.5 mg/kg - 60 seconds, 80% of people ready for intubation - can reverse with neostigmine in 2 minutes - lasts 15-20 minutes - acceptable rate of side effects - ? replacement for succinyl choline - has to be mixed but doesn't need refrigeration, ? ideal RSI drug for prehospital - acceptable cost - lots of histamine release and bronchospasm, withdrawn from market in early 2001 þ Medicolegal Notes from Ron Walls: - Baumgarten Anaesthesiology 1989 70 says must use neuromuscular blockers for intubation.
